If You Are Registering More Than One Person

Please complete a separate registration form for each person. This ensures that your medical records are correctly established and maintained from the start.

Please register yourself before registering any children under the age of 16. This identifies you as the primary contact and the responsible person for your children's medical treatment. This also links family members' records which is important for coordinated care and family history.
